View, complete, and submit our forms securely at your convenience—whether you're a new client starting therapy or an existing client updating personal or medical information.

These forms help us understand your needs, protect your privacy, and ensure a smooth start to your care. If you’re unsure which forms apply, our team is here to help.

Review the list of forms below and submit them at least 24 hours 

before your first appointment.

Client History Form

All clients are required to complete the Client History Form at least 24 hours prior to their first appointment. This form collects important background and contact information to help our team prepare for your visit and ensure your care starts smoothly.

→ Please have your insurance card(s) and a photo ID ready.

Client Contract & Consent for Treatment

This form outlines our policies and expectations for care, including attendance, fees, communication, confidentiality, and your rights as a client.

Coordination of Care

This form allows us to coordinate care with your medical or prescribing providers, such as psychiatrists or primary care physicians.

→  If you do not have a medical professional you’d like us to coordinate care with, please select “No” when prompted.

Payment Authorization

This form is required for all non-Medicaid clients and is completed through our secure client portal. You will receive a welcome email with instructions to set this up.

If you use an HSA card, we also request a backup card in case funds are temporarily unavailable.

Shared Custody & Financial Consent

This form is required only for minor clients when a legal custody agreement is in place. Each parent must complete their own form prior to the first appointment. SLC may request custody and financial documentation.

If financial responsibility is shared, each parent must have a valid card on file.

→ SLC does not mediate between parents. All scheduling, transportation, and financial coordination must be handled directly between parents.

Release of Information

Use this form to authorize communication with non-medical individuals or organizations, such as school staff, attorneys, probation officers, or family members.

→  A Release of Information is not required for routine uses such as billing, legal requirements, or treatment-related operations.
