Clients must have their registration packet completed at least 48 hours before their scheduled appointment, or the appointment will be canceled. Silver Linings Counseling also requires a credit card on file to charge copays, missed appointment fees, and account balances.

You will also be sent a payment authorization form before your first appointment (please check your spam folder for the invitation to join our client portal).
Additional Forms:
The Release of Information is for anyone you would like us to be able to communicate with for any reason. For example — a doctor, psychiatrist, specific person, school counselor, lawyer, probation officer, etc.
This form is not required.
The Shared Custody Consent is a required form for divorced, separated, and/or non-married parents/legal guardians or minors ONLY if a legal shared custody and/or financial agreement in place.
